INSTRUCTIONS:


DAYCARE, PRESCHOOL, ELEMENTARY AND MIDDLE SCHOOL:

Completed application and immunization forms may be mailed or delivered to the school office with the appropriate registration fee after approval from the Department Director or Principal. All applications are subject to availability.

SUMMER SAFARI:

Completed application may be mailed or delivered to the school with the appropriate registration fee. There is limited space available, please call the school office to verify if your child is registered for the week(s) requested.
