Student Registration Forms
INSTRUCTIONS:
DAYCARE, PRESCHOOL, ELEMENTARY AND MIDDLE SCHOOL:
Completed application and immunization forms may be mailed or delivered to the school office with the appropriate registration fee after approval from the Department Director or Principal. All applications are subject to availability.
SUMMER SAFARI:
Completed applications may be mailed or delivered to the school office with the appropriate registration fee. There is limited space available. Registration does NOT reserve a day or week in the program. Days and/or weeks must be paid in advance.
